As the named Activity Coordinator, by submitting this form for acceptance by Special Olympics Washington (SOWA), I understand that due to national Special Olympics regulations, legal regulations and insurance requirements SOWA has the right to accept or deny the fundraising activity. As the Activity Coordinator, I also understand SOWA has regulations, and process and procedures for fundraising activities, including but not limited to financial and auditing regulations. As the Activity Coordinator I will ensure the fundraising activity adheres to all SOWA policies and procedures. SOWA will notify the Activity Coordinator no later than one week from submission of this form of the acceptance or denial of the fundraising activity.
